What would be a functional medicine approach to these viral illnesses?

Dr. Amy Nett:  I was thinking rest, stress management, monolaurin, and coconut oil. Complicating factor for the spouse is high-risk LDL particle number. So LDL-P is over 2,500, small LDL-P, Apo B, LDL-C, adiponectin, pro-insulin. A1c is 5.9, so a little bit high, glucose 97 little bit high on a moderate carb diet. CRP 1.5, intermediate risk for fibrinogen, HSPL 2, oxidized LDL. Not sure of using monolaurin and/or coconut oil to fight potential viruses will negatively affect lipids. I have not seen Lauricidin negatively affect lipid profile. If this patient is really, really sensitive, so I’m assuming then this patient’s already on a Mediterranean Paleo diet. Oh, so, then you have the spouse is showing prediabetes. Yeah, I mean the hemoglobin A1c of 5.9 and glucose of 97, yeah, I mean the 5.9 is a little bit high. So should they do a low-carb Paleo or moderate-carb Paleo Mediterranean? Yeah, I wouldn’t worry about the monolaurin until you’ve switched them to a Mediterranean Paleo diet. And then you say a patient has high LDL particle number for over two years, under investigation for heavy metals and mold, but extreme stress in the past one-and-a-half years. Oh, yeah, and then what would I recommend?

 

Okay. So what I would think about doing, if there’s a lot of fatigue, first of all, have you checked cortisol? I’m guessing you already have, but double-check cortisol. Make sure this isn’t like an adrenal issue because of all that stress with everything. The other thing to remember is that chronic viruses can affect LDL. Remember that LDL can function in the immune response. So sometimes when we see chronic infection, that’s enough to raise LDL. So I wouldn’t worry about using monolaurin or coconut oil in treating a viral infection because treating that viral infection effectively might lower LDL. But that said, if you haven’t transitioned yet to a Mediterranean Paleo diet, then you don’t know how coconut oil affects him. So what I would do is say try a Mediterranean Paleo diet and remember in some versions of the Mediterranean Paleo diet for people who are only moderately affected by saturated fats, you can still include coconut oil, and then if their LDL-P remains high on a Mediterranean Paleo diet with coconut oil, then remove coconut oil also and go to a stricter Mediterranean Paleo diet. I would check cortisol before deciding whether or not you should do a low-carbohydrate diet because if their cortisol issue is contributing to this fatigue, then the last thing you want to do is go to low carbohydrate. So cortisol is the big missing information here.

 

In terms of a functional approach, it’s always great to do rest and stress management, but if the immune system is really out of balance I like what you’re doing in terms of the, what did you say here? Yeah, antiviral approach. Sorry. So if the immune system’s really out of balance, what I would think about is how to bring the immune system back into balance, and for an antiviral approach, use the Buhner antiviral book. So again, we’ve talked about Stephen Buhner a few times and mentioned that book, and he has a book called Herbal Antivirals. And so what you want to do is test the viruses to know what you should be treating. Because some viruses respond better to certain herbs. So for example we have sort of a chart part in MD HQ that’s specifically for HSV. And I’ve put together a combination of herbs that is specifically targeted to HSV.

 

If you’re going after CMV, you’re going to be using some different herbs. But what I’ll generally do when I’m treating a virus is I’m going to do general immune support. So that might be something like liposomal glutathione, liposomal vitamin C, zinc, selenium, I might use something like Transfer Factor from Researched Nutritionals. I sometimes use an herbal tincture from Woodland Essence that you can order directly from Woodland Essence and we do, what do we do in that one? Astragalus cordyceps and rhodiola, I believe in that formula for treating HSV. I substitute out the rhodiola for reishi. And then I tend to do additional antiviral herbs that are more targeted to the specific virus that I want to treat. I sometimes use the Byron White formulas. So again if it’s EBV or HHV-6, I’ll use the Byron White formula A-EB/H6. Yeah so I would just do the testing to figure out what virus you’re treating and then use the Buhner antiviral book to help you. And I wouldn’t worry about lurasidone. That is one I generally use for treating viruses, oh, along with apolactoferrin too. But I don’t think lurasidone in and of itself is going to to affect LDL-P. So hopefully that gives you sort of a starting place in terms of where to go.
