Trailblazers Grant Application

Are you a trailblazer?

A limited number of grants, offering up to 100% ($9,997) of tuition, will be awarded for the Fall 2021 ADAPT Health Coach Training Program.

These grants are motivated by the organization’s commitment to:

  1. Train underrepresented people who are creatively reducing healthcare disparities.
  2. Train students who have a plan to creatively reduce healthcare disparities in underserved communities.

Important dates:
  • Application opens: August 9, 2021
  • Application deadline: August 20, 2021
  • Notification of grant awards and waitlist: September 8, 2021
  • Deadline to accept a grant: September 13, 2021

  • Grant Qualifications

    Please ensure you are eligible to enroll in the program. Once finished, come back to the form and indicate your eligibility.
    The qualifications can be found here. Kresser Institute reserves the right to exclude any application from consideration that does not match the grant criteria.
  • Video and Written Submission

    We’re really excited to hear about your Trailblazer vision and how you plan to be a change agent for your community. Please share your detailed plan with us in a video of no more than 5 minutes, and thoroughly and thoughtfully complete the below form. The more details you include, the easier it will be for us to evaluate your plan.
  • Please record a brief video overview—no more than 5 minutes—of why you’d like to be considered for a Trailblazer grant, who you’d work with, and what your plan is to work with them, with the greatest focus being on your plan.

    Note: Use a service like YouTube, Vimeo, Dropbox, or CloudApp or another service to share your video at a publicly accessible URL. Make sure that your video can be accessed by the grant review committee (e.g., in YouTube, you can make it unlisted rather than private).

  • I acknowledge these Terms and Conditions upon applying for and accepting a scholarship from Kresser Institute:
    • I agree to complete all coursework and pass all certification requirements within 18 months of the course start date..
    • I agree to be fully on track with coursework in order to retain my grant. I understand recipients will be evaluated every 6 weeks at a check-in with the student advisor.
    • I will regularly share feedback about the program with the grant committee.
    • If I am behind on coursework, I will be given 30 days to catch up. If I fail to catch up during that time, I will lose access to the course.
    • I will submit a revised plan for how I will bring health coaching to an underserved community by the end of the final week of lessons.
